- Income Verification/ Eligibility Documentation (An example would be your 1040 Tax Form, W-2, Child Support, Unemployment Compensation)
- Photo ID of Parent/Guardian registering the child
- Birth Certificate
- Social Security card for the child
- Proof of Residence (CPS or SAWS bill or lease showing physical address in the name of the person enrolling)
- Federal Assistance (TANF, SSI)
- Insurance Coverage (For example, Medicaid, CIP, Personal Insurance)
- Physical Exam from Physician
- Dental Exam, if available
- Immunization Record
